E-MAIL ASSIGNMENT: Student Information Form
GEOG 3813, Geographic Information Systems (GIS) FALL 2008

All students in the class are REQUIRED to send the instructor the following information using e-mail via the Internet (using NSUnet at NSU or, if you prefer, a commercial Internet Service Provider--ISP). Print out this WWW page or jot down the required student information form for use when you log-on to the Internet to send e-mail. You may provide the following information by reference to its number or description.Remember, this e-mail assignment is worth 15 points (for a completed form submitted via e-mail). I must receive it no later than 5:00 p.m. on Tuesday, August 26, 2008. If you have questions or problems, please contact me.
Please supply the following information about your class section and yourself. Simply list the item number and give the required information.
1. Class number (this will be GEOG 3813)
2. Your full name (last name first)
3. The name you prefer to be called (such as Dr. Ziehr or Chuck or Mr. Ziehr)
4. Your year in school (Freshman, Sophomore, Junior, Senior, or Graduate)
5. Your major (even if only tentatively decided)
6. Your minor (if your program of study requires one)
7. Your LOCAL mailing address (including Zip Code)
8. Your LOCAL phone number (unless unlisted)
9. Your NSUnet e-mail address (this is the address I will use for this course, so please check your NSU e-mail at least once a week during this course)
10. Your HOME (permanent) mailing address (including Zip Code)
11. Your HOME phone number (unless unlisted)
12. The names and e-mail addresses of THREE other members of this class (ask the persons sitting next to you in class or other friends in class; the purpose is to encourage collaborative effort among students)
13. Please tell me an "interesting", "useful", "humorous", or "serious" (your choice) thing about yourself that would be helpful to me in getting to know you. Write at least two complete sentences.
14. List all other computer-intensive, college-level courses you have completed (preferably by title rather than number).
15. Do you have ready access to a personal computer other than those available at NSU? If so, please tell me a little about it?You must address all fifteen questions in order to receive full credit. If you prefer not to provide your address and/or phone number, please write "withheld" by that item number.
OPTIONAL -- Bonus (+3 points): Attach a digital photo of yourself to the e-mail (it must be in .jpg or .gif format). It will help me remember which name goes with which face in class!
You must address all fifteen questions in one e-mail in order to receive credit. I will grade the first e-mail that I receive from you, so make sure it is complete before you send it. If you prefer not to provide your address and/or phone number, please write "withheld" by that item number. You must include the information items in the body of your e-mail (not as an attachment). The optional digital photo is the only thing that may be an attachment.

I will respond to any e-mail that I receive from you, usually within 24 hours; so check to see if you get a response from me--if you don't, try sending your e-mail again.
