E-MAIL ASSIGNMENT: Student Information Form
All students in the class are REQUIRED to send the instructor the following information using e-mail via the Internet (using NSUnet at NSU or, if you prefer, a commercial Internet Service Provider--ISP). Print out this WWW page or jot down the required student information form for use when you log-on to the Internet to send e-mail. You may provide the following information by reference to its number or description.
Remember, this e-mail assignment is worth 15 points (for a completed form submitted via e-mail). I must receive it no later than 5:00 p.m. on Wednesday, January 22, 2003. If you have questions or problems, please contact me.
Please supply the following information about your class section and yourself. Simply list the item number and give the required information.

9. Your NSUnet e-mail address (and another e-mail address if you prefer to receive mail at it, but you MUST also provide your valid, working NSUnet userid). Please check your e-mail at least once a week during this semester.
10. Your HOME mailing (permanent) address
11. Your HOME phone number (unless unlisted)
12. The names and e-mail addresses of TWO other members of this class (ask the persons sitting next to you in class or other friends in class; the purpose is to encourage collaborative effort among students)
13. What other mapping-related and/or computer-oriented courses have you completed?
14. Please tell me an "interesting", "useful", "humorous", or "serious" (your choice) thing about yourself that would be helpful to me in getting to know you.
15. Please tell me your immediate career goal (just after graduation); this will help the instructor channel the course to meet specific, practical applications.
You must address all fifteen questions in one e-mail in order to receive full credit. If you prefer not to provide your address and/or phone number, please write "withheld" by that item number.
Bonus: You may earn three (+3) bonus points by inserting or attaching a digital picture of yourself. The picture will help me remember which name goes with which face. There is a scanner for student use in the back of the computer lab in the College of Education (EDUC 116); you may scan a regular photo there to create a digital file which can be inserted into or attached to the e-mail. The picture is completely optional, and you may receive full credit without it.

I strongly recommend that you send yourself a copy of this and all important e-mail, especially graded assignments (using the Cc: option). That way you'll have a record of your assignment and can check for transmission errors that may have occurred.
