E-MAIL ASSIGNMENT: Student Information Form

GEOG 3133, Principles of Economic Geography (11:00 TTh)    May Intersession 2008

Chuck Ziehr, Northeastern State University

All students in the class are REQUIRED to send the instructor the following information using e-mail via the Internet (using NSUnet at NSU or, if you prefer, a commercial Internet Service Provider--ISP). Print out this WWW page or jot down the required student information items for use when you log-on to the Internet to send e-mail. You must provide the following information by reference to its number or description. In this and all assignments in this course it is very important that you follow carefully all of the instructions.  Plan ahead; you will need assistance from your classmates on question #15.

Remember, this e-mail assignment is worth 15 points (for a completed form submitted via e-mail). I must receive it no later than 5:00 p.m. on Wednesday, May 14.  If you have questions or problems, please contact Dr. Ziehr.

Please supply the following information about your class section and yourself. Simply list the item number and give the required information.
1. Class number (this will be GEOG 3133)
2. Class meeting time and days (this will be 8:30 MTWThF)
3. Your full name (put your last name first; for example: Ziehr, Charles Thomas)
4. The name you prefer to be called (such as Chuck, Dr. Ziehr, or Mr. Ziehr)
5. Your year in school (FR, SOPH, JR, SR, or GRD)
6. Your current academic major (even if only tentatively decided)
7. Your current academic minor (even if only tentatively decided)
8. Your LOCAL mailing address (including Zip Code)
9. Your LOCAL phone number (unless unlisted)
10. Your NSUnet e-mail address.  Your NSU e-mail address is your userid followed by @nsuok.edu; for example, mine is ziehr@nsuok.edu.  All e-mail related to this course will be sent to your NSU e-mail address; please check your e-mail at least once a week during this course.
11. Your HOME  (permanent) mailing address (including Zip Code)
12. Your HOME phone number (unless unlisted)
13.  Name of the high school from which you graduated and the year you graduated
14.  County (or parish or equivalent political unit if outside the U.S.) and State in which your high school was located  (if you're an international student, please also list your home country)
15.  The names and e-mail addresses of THREE other members of this class (ask the persons sitting around you in class or other friends in class; the purpose is to encourage collaborative effort among students)

16.  OPTIONAL -- Bonus (+2 points):   Please tell me something "interesting", "useful", "humorous", or "serious" (your choice) about yourself that would be helpful to me in getting to know you.  You must write at least two complete sentences.

17.  OPTIONAL -- Bonus (+3 points):  Attach a digital photo of yourself to the e-mail (it must be in .jpg or .gif format).  It will help me remember which name goes with which face!

You must address all of the first fifteen information items in one e-mail in order to receive credit. I will grade the first e-mail that I receive from you, so make sure it is complete before you send it. If you prefer not to provide your address and/or phone number, please write "withheld" by that item number.  You must include the information items in the body of your e-mail (not in an attached document).  The optional digital photo is the only thing that may be an attachment.

Send the above information to the instructor at the following address:    ziehr@nsuok.edu

I strongly recommend that you send yourself a copy of this and all important e-mail, especially graded assignments (using the Cc: or Bcc: option).  That way you'll have a record of your assignment and can check for transmission errors that may have occurred.  I will respond to any e-mail that I receive from you, usually within 24 hours; so check to see if you get a response from me--if you don't, try sending your e-mail again.

Please feel free to call on me if I can help you in any way. Best wishes for a successful learning experience!
