TPS Health Service Forms TPS Immunization Requirements	Clinic Information Medication is given to students when proper forms are completed in the clinic.  A note from the parent is not acceptable.  Legal forms are required to dispense medication.  Cough drops may not be carried with the child.  They are considered a medication and will only be distributed with appropriate forms through the clinic. Any over the counter medications and/or prescriptions must come in the original container (where dosage is evident). Students are sent home in cases of vomiting or fever over 100 degrees. Please have up to date medical information noted in your child's records.  Food allergies, medications, and emergency contact information must be current and updated as needed.
