  2. Refer to the Online Course Development Evaluation Instrument for a complete definition of each criterion.
  3. Below each criterion, state how you met the criterion.  After you complete the criteria descriptions, submit the application with your chair and dean’s signature to:  Distance Learning Committee, NET 610 or you may fax the form to x2382.
  4. Applications are due by October 1 of each academic year. Notification for awards will be made by May 1 of each academic year.
