  1. Describe how you addressed each of the following criteria in the table below in your online course development.
  2. Refer to the Online Course Development Evaluation Instrument for a complete definition of each criterion.
  3. Below each criterion, state how you met the criterion.  After you complete the criteria descriptions, submit the application with your chair and dean’s signature to:  Distance Learning Committee, NET 610 or you may fax the form to x2382.
  4. Applications are due by October 30 of each academic year. Notification for awards will be made by December 1 of each academic year.
