Applicants can now submit applications in paper form to the Graduate Office in Tahlequah by mail, office mail, or by dropping it off at the desk as before; but they can also email the application form as an attachment to an email to irb@nsuok.edu . For email applications, the signature page with original signatures must still be mailed to the office. Email submissions are encouraged to speed up initiation of the review process and because digital files are easier to distribute and process. Final approval for emailed applications can not be given without original signatures of all researchers and/or faculty sponsor.

SUBMIT THREE COPIES, AT LEAST ONE COPY MUST HAVE ORIGINAL SIGNATURES

Early submissions are encouraged. Upon receipt of the application, whether by mail or email, the Chair of the IRB will review if the application is exempt, whether it can be expedited, or if it requires full Board review. Exempt applications will receive immediate approval. Expeditable applications will be assigned to two reviewers in between meetings as possible. Full Board reviews need to be submitted by the deadlines below in order to be placed on the agenda for the meeting.
